What this test is for
Homocysteine is an amino acid the body normally clears using vitamins B12, B6, and folate, so the level reflects how well that methylation pathway is working. High levels point to B12 or folate deficiency, and are also linked to impaired kidney function, hypothyroidism, advanced age, and certain medications. Elevated homocysteine is an independent risk factor for cardiovascular disease, with higher levels seen in heart disease, stroke, peripheral vascular disease, and thromboembolic disease; rarer inherited enzyme defects (such as cystathionine-beta-synthase deficiency or an MTHFR variant) can also raise it. Lowering the number with B vitamins does not by itself guarantee lower cardiovascular risk, so treat it as a marker rather than a target to chase.
If the goal is specifically to sort out a suspected B12 deficiency, the Methylmalonic Acid and Homocysteine Panel pairs this marker with MMA, which together separate B12 deficiency from folate deficiency more cleanly than homocysteine alone.
