Pre-test considerations
No fasting required. Seafood can transiently raise arsenic and mercury, so avoid fish and shellfish for several days before collection for the most interpretable result. The sample must be collected in an acid-washed or metal-free container to avoid contamination. Note any supplements or recent imaging contrast, since these can affect metal levels.

What this test is for
This panel measures arsenic, lead, mercury, and cadmium in urine, the four toxic metals most often picked up from contaminated water, seafood, industrial work, old paint, tobacco smoke, and some imported products. Urine reflects recent and ongoing exposure, and results are reported as a ratio to creatinine to correct for how dilute or concentrated the sample is. Elevated levels are linked to fatigue, headaches, abdominal pain, numbness or tingling, cognitive changes, kidney strain, and anemia, though symptoms depend heavily on which metal and how much exposure occurred. Cadmium in particular accumulates with long-term tobacco use and certain occupational exposures, which is why it is included here.

Urine creatinine tracks a waste product muscle breaks down at a steady rate, which makes it the reference point for judging whether a urine sample is concentrated or diluted. Labs use it to normalize other urine markers, most importantly albumin, into a ratio (ACR) that actually reflects kidney filtering rather than how much water you drank that morning. On its own it is rarely the number you care about, but pair it with albumin and it becomes one of the earliest ways to catch kidney strain from diabetes, high blood pressure, or high-protein training loads before symptoms show up.

Where do I get tested?
