Pre-test considerations
No fasting required. A standard venous blood draw collected in a metal-free (royal blue-top) tube. Avoid seafood for several days before testing, since fish and shellfish can transiently raise arsenic and mercury and produce results that look elevated without reflecting harmful exposure. Note any supplements, as some raise cobalt or arsenic.

What this test is for
This panel measures five toxic and trace metals in whole blood: arsenic, cadmium, cobalt, lead, and mercury. Blood levels reflect recent or ongoing exposure rather than long-term body burden, which is what makes this the right test when you suspect a current source: lead from old paint, pipes, or hobbies; mercury from large predatory fish or dental work; arsenic from contaminated water or seafood; cadmium from smoking or industrial dust; cobalt from metal-on-metal joint implants or supplements. Symptoms of metal toxicity are nonspecific and overlap heavily, including fatigue, headaches, abdominal pain, numbness or tingling, brain fog, and tremor, so testing is how you confirm or rule out a metal as the cause. This five-metal version adds cobalt and cadmium to the standard arsenic, lead, and mercury group, useful if you have a joint implant or a smoking and occupational exposure history; if you only need the core toxic metals, the three- or four-metal blood panels cover those, and the urine panel is the better choice for assessing cumulative arsenic and cadmium exposure.

Blood mercury measures total mercury exposure, mostly from methylmercury in seafood, which accumulates in blood, kidney, and brain tissue rather than being cleared quickly. High levels track with frequent consumption of tuna, swordfish, or other large predatory fish, and can contribute to fatigue, brain fog, tingling, headaches, or memory complaints. It's a useful check if you eat a lot of fish, take fish oil or supplements of unclear purity, or want a baseline before increasing seafood intake for its cardiovascular and cognitive benefits.
