Pre-test considerations
No fasting required. Seafood can raise urine arsenic for a few days, so avoid fish, shellfish, and seaweed for about 48 to 72 hours before collection. Avoid gadolinium-based MRI contrast and iodine contrast before testing, since these interfere with metal measurement. The sample must be collected in an acid-washed or metal-free container to avoid contamination.

What this test is for
This panel measures six toxic and trace metals in urine (arsenic, cadmium, mercury, thallium, cobalt, and lead) alongside creatinine, which normalizes the result for how dilute the sample is. Urine is the standard way to assess recent or ongoing exposure to metals that the kidneys clear, and elevated levels point toward an environmental, occupational, dietary, or supplement source worth tracing. Heavy metal exposure can contribute to fatigue, headache, brain fog, numbness or tingling, abdominal pain, and kidney problems, though symptoms are nonspecific and overlap with many other conditions. Cobalt is also useful for people with metal-on-metal joint implants, where wear can raise levels.

Urine creatinine tracks a waste product muscle breaks down at a steady rate, which makes it the reference point for judging whether a urine sample is concentrated or diluted. Labs use it to normalize other urine markers, most importantly albumin, into a ratio (ACR) that actually reflects kidney filtering rather than how much water you drank that morning. On its own it is rarely the number you care about, but pair it with albumin and it becomes one of the earliest ways to catch kidney strain from diabetes, high blood pressure, or high-protein training loads before symptoms show up.
