What this test is for
GAD65 antibodies are markers of the autoimmune process that destroys insulin-producing beta cells in type 1 diabetes. A positive result points to autoimmune diabetes rather than type 2, which matters because adult-onset cases are misdiagnosed often: GAD65 is the most common autoantibody in latent autoimmune diabetes in adults (LADA) and frequently the first to appear in childhood-onset disease. Beyond diabetes, GAD65 antibodies help confirm several neurological conditions, including stiff-person syndrome, autoimmune encephalitis, autoimmune ataxia, and autoimmune epilepsy. One important limit: staging presymptomatic type 1 diabetes requires at least two islet autoantibodies, so GAD65 alone screens but does not fully classify, and the ADA recommends testing all four islet antibodies (GAD65, IAA, ZnT8, IA-2) when ruling in early disease. A positive GAD65 result is the usual trigger to expand testing and reassess how diabetes is being managed.
