Pre-test considerations
Eat a normal diet with at least 150 grams of carbohydrate daily for three days before the test, then fast 8 to 14 hours (water only). Stay seated and at rest during the test, since activity and stress shift the results. Do not have a GTT during acute illness, infection, or surgery, or while on corticosteroids. Where possible, pause nonessential medications that affect glucose for three days beforehand, after checking with your clinician.

What this test is for
The glucose tolerance test measures blood glucose while fasting and again two hours after drinking a standard 75-gram glucose solution, the WHO protocol for diagnosing diabetes and impaired glucose tolerance. A fasting value alone can look normal in people whose problem only shows up under a glucose load, which is exactly what the two-hour reading captures. It is most useful when fasting glucose is borderline, when there is unexplained high triglycerides, neuropathy, or glycosuria without high blood sugar, and to evaluate carbohydrate metabolism in pregnancy. Impaired glucose tolerance also carries increased cardiovascular risk, and 1 to 5 percent of people with it progress to overt diabetes each year.
This is the two-specimen version (fasting plus two-hour). The separate gestational version adds a one-hour draw for pregnancy screening, and a simple fasting glucose is the right test if you only need a single resting reading.
