Anti-glomerular basement membrane (anti-GBM) antibodies are autoantibodies that target the basement membrane lining the filtering units of the kidneys and the air sacs of the lungs. When present, they cause rapidly progressive glomerulonephritis and pulmonary hemorrhage, the combination known as Goodpasture's syndrome. The test is ordered when someone has unexplained acute kidney injury, blood in the urine, or coughing up blood, often to distinguish anti-GBM disease from other causes of rapidly progressive kidney damage and vasculitis. A positive result is clinically urgent because anti-GBM disease moves fast and treatment outcomes depend heavily on how early it is caught. It is typically interpreted alongside kidney function, urinalysis, ANCA testing, and frequently a kidney biopsy.
