Pre-test considerations
Fasting is not required for this screen. Drink the full 50-gram glucose solution, then have blood drawn exactly one hour later. Stay seated and do not smoke during the waiting hour, since both can affect the result. The collection time should be labeled as the one-hour interval.

This is the one-step glucose challenge used to screen for gestational diabetes, typically at 24 to 28 weeks of pregnancy. You drink a 50-gram glucose solution and have blood drawn exactly one hour later to see how well your body clears the load. A result above roughly 140 mg/dL is a positive screen, which means further testing, not a diagnosis: it identifies about 80% of women with gestational diabetes and flags who should go on to the diagnostic 100-gram, three-hour test on a separate day. Gestational diabetes often has no symptoms, which is why screening is routine rather than symptom-driven. A normal result is reassuring and usually closes out the question for that pregnancy.
