Pre-test considerations
Overnight fasting is required, preferably 12 hours or more. Acid-suppressing drugs, especially proton pump inhibitors, raise gastrin and are a common cause of falsely high results, so discuss timing with your clinician before testing. Biotin supplements can distort the assay: separate standard doses by at least 8 hours and high doses by at least 72 hours from the draw. Interpret results in light of gastric acid secretion, since elevated gastrin alone is not specific.

What this test is for
Gastrin is a hormone made by G cells in the stomach lining that stimulates gastric acid production, antral motility, and secretion of pepsin and intrinsic factor. The main reason to measure it is to investigate severe or recurrent peptic ulcers, unexplained chronic diarrhea, and steatorrhea, where markedly high levels point toward Zollinger-Ellison syndrome and gastrin-secreting tumors (gastrinomas). Levels above 1000 pg/mL with acid hypersecretion essentially confirm Zollinger-Ellison syndrome, but moderate elevations have many causes, including antral G-cell hyperplasia, atrophic gastritis, and acid-suppressing medications like proton pump inhibitors. Because 15 to 26 percent of Zollinger-Ellison cases occur with multiple endocrine neoplasia type 1, an elevated result is sometimes the first clue to a broader endocrine picture. When fasting gastrin is high but not diagnostic, a secretin stimulation test is the usual next step to distinguish gastrinoma from other causes.
