GAD65 is an enzyme inside nerve cells that helps make GABA, the brain's main inhibitory neurotransmitter. When the immune system makes antibodies against it, that signaling is disrupted, and high antibody levels are linked to a cluster of neurological conditions: stiff-person syndrome (progressive muscle rigidity and painful spasms), subacute cerebellar ataxia (worsening balance, coordination, and gait), and some forms of autoimmune encephalitis and refractory epilepsy. This test measures anti-GAD65 antibodies in serum, which helps clarify whether an unexplained movement, balance, or seizure disorder has an autoimmune basis. The neurological syndromes are associated with high antibody titers; low or modest levels are also seen with type 1 diabetes and other autoimmune conditions, so the result is interpreted alongside symptoms and titer.
