Pre-test considerations
Fasting is not required for most of this panel. Fast 12 to 14 hours if triglycerides are a specific concern, such as evaluating familial hypercholesterolemia, early heart disease, very high triglycerides, or pancreatitis, since triglycerides rise after eating. Recent illness or infection can transiently raise GlycA, so avoid testing when acutely unwell.

This panel pairs a standard lipid profile (total cholesterol, HDL-C, LDL-C, triglycerides, and non-HDL cholesterol) with three markers that add information routine lipids miss. ApoB counts the atherogenic particles that actually drive plaque, and two people with the same LDL can carry very different particle burdens. GlycA is a composite NMR measure of systemic inflammation that integrates several acute-phase proteins, giving a more stable read than hs-CRP and requiring only a single measurement rather than two readings two weeks apart. The Diabetes Risk Index is an NMR-derived score from 1 to 100 that combines a lipoprotein insulin-resistance pattern with branched-chain amino acids to estimate the risk of developing type 2 diabetes, often flagging trouble in people whose glucose and A1c still read normal.
