What this test is for
Estrone sulfate is the most abundant circulating estrogen in non-pregnant people. It is biologically inactive itself but serves as a stable reservoir that converts to estrone and then to estradiol, the most potent estrogen. Because it circulates at much higher concentrations than estradiol, it can be measured accurately in situations where estradiol is very low, such as in men, children, and postmenopausal women, which is its main practical advantage. Levels are raised by exogenous estrogens, including oral contraceptives, hormone replacement therapy, and conjugated equine estrogens, and lowered by aromatase or steroid sulfatase inhibitors, so current medications strongly shape the result. It has also been studied as a marker in estrogen-sensitive cancers and to monitor response to aromatase inhibitor therapy, though that use is not yet standardized.
