Pre-test considerations
No fasting required, and timing during the day does not matter. Stay on a normal gluten-containing diet before testing: removing gluten lowers these antibodies and can produce a falsely negative result, so testing is most reliable while gluten is still in the diet.

What this test is for
Deamidated gliadin peptide IgG antibodies form when the immune system reacts to gliadin, one of the main proteins in gluten. Elevated levels point toward celiac disease and related gluten-sensitive conditions like dermatitis herpetiformis, and they help investigate chronic diarrhea, bloating, weight loss, iron deficiency, fatigue, and faltering growth. The deamidated version is more accurate than older standard gliadin antibody assays because it targets the specific epitopes celiac antibodies actually bind. It is most useful in younger children and in people with low IgA, where the standard tissue transglutaminase IgA test can miss cases.
This is the IgG-only form. The combined IgG and IgA version adds the IgA antibody, which is the more sensitive class in most adults, so the paired test is the better default unless IgG alone is what you need.
