Pre-test considerations
No fasting required. Collection technique is what matters: at 8 AM, void and discard that first sample, then collect every void for the next 24 hours including the final one at 8 AM the next morning. Keep the container refrigerated during collection and record the exact start and finish times, since missed voids or unrecorded times make the result unreliable. Maintain your usual hydration; avoid an unusually heavy meat meal or intense exercise just before, as both can transiently shift creatinine output.

This test measures the total amount of creatinine in a 24-hour urine collection, which is used two ways: as the urine half of a creatinine clearance calculation to estimate how well the kidneys filter, and as a quality check confirming a 24-hour collection was complete (output tracks closely with muscle mass, so an unexpectedly low total often signals missed voids). It is most informative paired with a same-period blood creatinine for clearance, or alongside a 24-hour analyte like protein, albumin, or calcium to express that result per gram of creatinine. Because daily excretion scales with muscle mass, values differ by sex, age, and body size and are interpreted against those expectations rather than a single universal cutoff.

Urine creatinine tracks a waste product muscle breaks down at a steady rate, which makes it the reference point for judging whether a urine sample is concentrated or diluted.
