Pre-test considerations
No fasting required. Timing is the critical variable: because CK-MB follows a sharp rise-and-fall curve after cardiac injury, it is typically drawn on presentation and repeated at set intervals over the following hours rather than as a single reading. Strenuous exercise, muscle trauma, and intramuscular injections before the draw can raise both total CK and CK-MB.

What this test is for
CK-MB is an isoenzyme of creatine kinase concentrated in heart muscle, where it makes up a much larger share of total CK than in skeletal muscle. This test measures the CK-MB protein directly by mass (ng/mL), the form preferred for cardiac evaluation, and reports it alongside total CK so the proportion coming from the heart can be assessed. After a heart attack, CK-MB typically rises within four to six hours, peaks at 12 to 24 hours, and returns to baseline within 24 to 48 hours, which is why serial measurements over hours tell you far more than a single draw. It is not perfectly heart-specific: severe exercise, muscular dystrophy, polymyositis, and skeletal muscle injury can also raise it, so the value is read against total CK and the clinical picture. This is the mass-based measurement; the separate CK-MB Activity test reports enzyme activity in U/L and is used where that assay is preferred.
