Pre-test considerations
Avoid strenuous exercise for at least 24 to 48 hours before the draw, since intense activity, even a hard workout the day before, can sharply raise CK on its own. Recent intramuscular injections, surgery, or muscle trauma also elevate it. Fasting is not required.

What this test is for
Creatine kinase (CK) is an enzyme concentrated in skeletal muscle and heart tissue, and the total level rises when muscle cells are damaged and release it into the blood. Elevated CK is seen with intense or unaccustomed exercise, direct muscle trauma, rhabdomyolysis, inflammatory myopathies such as polymyositis and viral myositis, muscular dystrophies, hypothyroidism, and statin-related muscle injury, and it is sometimes the first abnormal finding in a neuromuscular disorder. It is commonly checked when investigating muscle pain, weakness, dark urine, or cramping, and to monitor people on statins or fibrates who develop muscle symptoms. This is the total CK test, which tells you how much enzyme is in circulation but not where it came from; the separate CK-MB and CK isoenzyme tests break the total into its heart and skeletal-muscle fractions when the source needs to be pinned down. For tracking training load, recovery, or a known muscle condition, total CK is the right default.
