Pre-test considerations
Timing is the whole point: this is a late-day draw, so collect in the evening and note the exact collection time and date. High-dose biotin (vitamin B7) supplements can distort the result, so stop biotin at least 72 hours before the draw. Acute physical or emotional stress and recent illness raise cortisol and can cloud interpretation.

What this test is for
Cortisol is the adrenal stress hormone that follows a strong daily rhythm: it peaks in the early morning and falls to its lowest point in the evening. This test measures the late-day (PM) trough, which is most useful for evaluating that rhythm rather than total output. A PM level that stays high instead of dropping is a classic sign of Cushing's syndrome (cortisol excess), while consistently low cortisol points toward adrenal insufficiency (Addison's disease). It pairs naturally with the AM (peak) measurement, since the difference between morning and evening levels is often more telling than either value alone.
This is the evening-trough version of the serum cortisol test. The standard Cortisol and Cortisol, AM tests capture a single timed level, the saliva and 24-hour urinary free cortisol tests measure unbound cortisol, and the ACTH stimulation test checks how the adrenal glands respond to a challenge.
