Pre-test considerations
No fasting required. For the 24-hour collection, void and discard the first morning sample, then collect all urine through the same time the next morning into a preservative-free container; an incomplete collection lowers the measured total. Note any copper-containing supplements or recent chelation treatment, since both affect excretion.

What this test is for
Urine copper measures the amount of copper cleared into the urine, reported as a concentration, a 24-hour total, and a copper-to-creatinine ratio to correct for how dilute the sample is. Elevated urinary copper is a hallmark of Wilson disease, an inherited disorder where copper accumulates in the liver and brain, and it also rises with biliary cirrhosis, chronic active hepatitis, and copper overexposure. The 24-hour collection is the more reliable form for diagnosis and for monitoring chelation therapy, since it captures total daily excretion rather than a single snapshot. Symptoms that prompt copper evaluation cluster around the liver and nervous system: unexplained liver enzyme abnormalities, tremor, difficulty with coordination, behavioral or mood changes, and in younger people, early neurological signs.
This urine test reflects copper being excreted, which complements the serum copper test that measures circulating levels; in Wilson disease, serum copper and ceruloplasmin are often low while urine copper is high, so the two are read together.

Urine creatinine tracks a waste product muscle breaks down at a steady rate, which makes it the reference point for judging whether a urine sample is concentrated or diluted. Labs use it to normalize other urine markers, most importantly albumin, into a ratio (ACR) that actually reflects kidney filtering rather than how much water you drank that morning.
