Coenzyme Q10 (ubiquinone) sits in the inner mitochondrial membrane, where it carries electrons through the chain that produces ATP, and it doubles as a lipid-soluble antioxidant protecting cell membranes and lipoproteins. This test measures total CoQ10 in serum, covering both the oxidized (ubiquinone) and reduced (ubiquinol) forms. Levels drop with statin therapy, because statins inhibit the same HMG-CoA reductase enzyme used to build CoQ10, and the decline is dose related and reverses after stopping the drug. CoQ10 is also supplemented for chronic heart failure, hypertension, migraine, and male infertility, so testing is most useful for confirming low status or checking whether supplementation is moving your level. Because CoQ10 rides on LDL particles, serum levels track with total and LDL cholesterol, and results are usually interpreted as a CoQ10-to-lipid ratio rather than a raw number.
