What this test is for
Cobalt is a trace element found in the diet and built into vitamin B12, but in higher amounts it becomes toxic. A blood cobalt test measures recent and ongoing exposure, most often to check people with a metal-on-metal hip replacement (cobalt-chromium implants can shed cobalt ions as they wear) or those with industrial exposure to cobalt dust, alloys, or hard-metal manufacturing. Elevated cobalt is linked to cardiomyopathy, thyroid dysfunction, neurological symptoms, and vision or hearing changes, so it is used both to confirm exposure and to monitor whether levels are rising or falling over time. Note that blood reflects current exposure; about 85% of absorbed cobalt is cleared in urine, so urine testing is sometimes paired with it for a fuller exposure picture.
