Pre-test considerations
No fasting required. Collect only loose or watery stool, since testing formed stool is not appropriate and can detect harmless carriage. The specimen must be kept frozen for transport (room-temperature samples are not acceptable), so plan collection close to drop-off or prompt freezing. If you have recently taken antidiarrheal medication, note it, as it can affect stool consistency and sample quality.

What this test is for
Clostridioides difficile is a spore-forming bacterium that can be part of normal gut flora, but when it overgrows (most often after antibiotics disrupt the rest of the microbiome) it produces toxins that cause frequent, foul-smelling, watery stools, cramping, and colitis. This test uses a cell-culture cytotoxicity assay to detect toxin B in stool, the toxin responsible for the tissue damage seen in C. diff infection and pseudomembranous colitis. A positive result means active toxin-producing infection rather than harmless carriage, which is why the assay looks for the toxin itself, not just the organism. It is most relevant when diarrhea follows recent antibiotic use, a hospital or care-facility stay, or a course of acid-suppressing medication. Testing is appropriate for unexplained, ongoing diarrhea and not for formed stool, since carriers without symptoms can test positive without needing treatment.

Where do I get tested?
