Pre-test considerations
No fasting is required. Note recent or current pregnancy and any estrogen-containing oral contraceptives, since both raise ceruloplasmin. Active inflammation or infection can also elevate the result, because ceruloplasmin behaves as an acute-phase reactant. For a Wilson's disease workup, serum copper is usually measured alongside it.

What this test is for
Ceruloplasmin is the main copper-carrying protein in blood, produced by the liver, and it reflects how the body incorporates and transports copper. Low levels are the hallmark of Wilson's disease, where copper cannot be loaded into the protein and instead accumulates in the liver and brain, causing liver dysfunction and neurological symptoms like tremor and coordination problems. It also drops with significant protein loss (nephrotic syndrome, malabsorption, advanced liver disease) and in Menkes syndrome. Because ceruloplasmin behaves as a slow acute-phase reactant, it rises with inflammation, infection, some cancers, pregnancy, and estrogen-containing oral contraceptives. A normal result does not rule out Wilson's disease, so it is interpreted alongside serum copper rather than alone.
