Pre-test considerations
No fasting required. The antibody portion is only accurate while you are eating gluten regularly, so do not start a gluten-free diet before testing; going gluten-free lowers antibody levels and can produce a false negative. The HLA genetic portion is unaffected by diet and is valid even if you are already gluten-free.

What this test is for
Celiac disease is an autoimmune reaction to gluten that damages the small intestine and drives symptoms like chronic diarrhea, bloating, weight loss, fatigue, anemia, and, in some people, no gut symptoms at all. This panel pairs the serology used to detect active disease (tissue transglutaminase IgA and IgG, deamidated gliadin IgA and IgG, and endomysial IgA) with HLA-DQ2 and HLA-DQ8 genotyping. The antibodies reflect an active immune response to gluten and are the first-line markers when celiac is suspected; the genetics tell you whether you carry the alleles that make celiac possible. Because nearly everyone with celiac carries DQ2 or DQ8, a negative genetic result effectively rules the disease out and is especially useful when antibody results are unclear or you are already gluten-free.

Endomysial IgA antibodies form when your immune system attacks the connective tissue supporting your gut lining, a hallmark response triggered by gluten in celiac disease. A positive result is highly specific for celiac disease, especially useful if you're investigating chronic bloating, diarrhea or constipation, unexplained weight loss, iron or B12 deficiency, or fatigue that doesn't resolve despite normal-looking meals. It's typically run alongside tissue transglutaminase IgA to confirm a diagnosis before committing to a gluten-free diet, since going gluten-free first can mask the antibody response and cloud the result.

This test looks for IgG antibodies your immune system builds against gliadin, a protein found in wheat, barley, and rye. A positive result signals an immune reaction to gluten, useful for investigating bloating, diarrhea, fatigue, joint pain, brain fog, or skin rashes that flare after eating bread or pasta. It's often run alongside tTG-IgA and total IgA to build a full picture of gluten sensitivity or celiac disease, especially when the more common tTG test comes back ambiguous.

Where do I get tested?
Choose an available partner lab at checkout. If your cart contains multiple blood tests, Mito helps consolidate the eligible tests at one lab when possible. At-home kits and scans follow the collection or appointment process shown on their product page.
