Pre-test considerations
No fasting required. Stay on a normal gluten-containing diet until your blood is drawn, since cutting gluten lowers these antibodies and can produce a false negative. If you are already gluten-free, results reflect dietary compliance rather than ruling celiac in or out.

What this test is for
This panel screens for celiac disease, an immune reaction to gluten in genetically susceptible people that damages the small intestine. It measures tissue transglutaminase IgA (tTG IgA), the primary antibody with roughly 95% sensitivity and specificity, alongside total IgA to catch the 2-3% of celiac patients who are IgA deficient and would otherwise produce a falsely negative tTG, plus endomysial IgA (EMA), a highly specific antibody used to confirm a positive tTG. It is worth checking when symptoms point toward celiac: chronic diarrhea, bloating, gas, weight loss, fatigue, or unexplained iron-deficiency anemia, especially with a family history of the disease.
This is the standard three-marker workup; the single tTG IgA test is just the screening antibody, and the Extended Panel adds deamidated gliadin and IgG-based antibodies for cases of confirmed IgA deficiency or strong suspicion despite negative IgA results. Because these antibodies fall on a gluten-free diet, the panel is also used to track dietary compliance after diagnosis.
