Pre-test considerations
No fasting required. For an accurate result you must be eating gluten regularly in the weeks before testing, since cutting gluten lowers these antibodies and can produce a falsely normal result. If you are testing to monitor a gluten-free diet, antibodies fall gradually, so allow several months between tests.

What this test is for
This panel measures three antibodies the immune system produces in response to gluten in people with celiac disease: gliadin IgG, gliadin IgA, and endomysial IgA with titer. Celiac is an autoimmune reaction to gluten that damages the small intestine and is commonly investigated when symptoms like chronic diarrhea, bloating, abdominal pain, unexplained weight loss, iron-deficiency anemia, or fatigue point toward malabsorption. The endomysial IgA antibody is highly specific for celiac, so it adds diagnostic weight beyond the gliadin antibodies alone, while the gliadin IgG is useful when someone is IgA deficient. The result is also used to track adherence to a gluten-free diet, since antibody levels fall as the gut heals.
The separate Gliadin Antibodies (IgG, IgA) panel covers only the two gliadin markers; this version adds the more specific endomysial antibody, making it the stronger first-line screen.

Endomysial IgA antibodies form when your immune system attacks the connective tissue supporting your gut lining, a hallmark response triggered by gluten in celiac disease. A positive result is highly specific for celiac disease, especially useful if you're investigating chronic bloating, diarrhea or constipation, unexplained weight loss, iron or B12 deficiency, or fatigue that doesn't resolve despite normal-looking meals. It's typically run alongside tissue transglutaminase IgA to confirm a diagnosis before committing to a gluten-free diet, since going gluten-free first can mask the antibody response and cloud the result.

This test looks for IgG antibodies your immune system builds against gliadin, a protein found in wheat, barley, and rye. A positive result signals an immune reaction to gluten, useful for investigating bloating, diarrhea, fatigue, joint pain, brain fog, or skin rashes that flare after eating bread or pasta. It's often run alongside tTG-IgA and total IgA to build a full picture of gluten sensitivity or celiac disease, especially when the more common tTG test comes back ambiguous.
