Pre-test considerations
No fasting required, and timing during the day does not matter. If you have had a recent infection or are on medication known to induce these antibodies, mention it, since transient positives are common. Because diagnosis depends on persistence, an isolated positive should be confirmed on a second sample at least 12 weeks later.

What this test is for
Anticardiolipin antibodies are autoantibodies that target phospholipid-binding proteins and are one of the markers used to evaluate antiphospholipid syndrome (APS), an autoimmune clotting disorder. This test measures the IgM isotype, which is associated with venous thrombosis and is checked when investigating unexplained clots, stroke at a young age, low platelets, or recurrent miscarriage. A single positive result does not diagnose APS: these antibodies are common in the general population, often transient, and can be triggered by infection, other autoimmune disease, malignancy, or certain drugs, so a diagnosis requires a medium-to-high titer confirmed on a repeat test at least 12 weeks apart.
This is the IgM-specific form. The separate IgG cardiolipin test carries the stronger association with both arterial and venous thrombosis and pregnancy loss, the combined IgG/IgM and IgA/IgG/IgM panels test multiple isotypes together, and the full antiphospholipid panel adds beta-2 glycoprotein I antibodies. For a complete APS workup, IgG and IgM are usually tested together alongside lupus anticoagulant.
