Pre-test considerations
No fasting required, and timing during the day does not matter. Note any anticoagulant or recent illness, since infections and some medications can transiently raise these antibodies. Because APS criteria require a repeat positive at least 12 weeks apart, a single result is a starting point, not a diagnosis.

What this test is for
Anticardiolipin antibodies (ACA) target phospholipids on cell membranes and are a core marker for antiphospholipid syndrome (APS), an autoimmune clotting disorder linked to stroke, heart attack, venous thrombosis, low platelets, and recurrent pregnancy loss. This panel measures all three isotypes (IgG, IgM, and IgA), with moderate-to-high IgG most strongly associated with both arterial and venous clots and recurrent miscarriage. ACA are common and often transient in the general population, including in normal pregnancy, so a single positive does not diagnose APS: the criteria require a medium or high titer confirmed on two occasions at least 12 weeks apart, read alongside your clotting and pregnancy history.
This three-isotype version adds IgA to the standard IgG and IgM pair, useful when APS is suspected but IgG and IgM are negative. For a full APS workup, the Antiphospholipid Antibody Panel pairs these with beta-2 glycoprotein 1 antibodies, and lupus anticoagulant testing is typically run as well, since ACA is more sensitive but lupus anticoagulant is more specific.
