Pre-test considerations
No fasting required. CA 19-9 is most informative when tracked serially under similar conditions, so consistency between draws matters. Benign causes such as bile duct obstruction, pancreatitis, and liver disease can elevate the result, which is why it is interpreted alongside imaging and clinical findings.

What this test is for
CA 19-9 is a protein shed by certain gastrointestinal tumors, most notably cancers of the exocrine pancreas, and also bile duct, liver, and colorectal cancers. Its primary role is serial monitoring: in someone with a confirmed cancer that produces CA 19-9, a rising or falling level tracks disease status and response to treatment. It is not a screening test for healthy people, because levels also rise with benign conditions such as bile duct obstruction, pancreatitis, and liver disease, and because tumor size does not correlate cleanly with the number. Levels above 10,000 units/mL almost always indicate distant metastasis. This is distinct from the related tumor markers in our catalog: CA 125 is used mainly for ovarian cancer, and CA 27.29 for breast cancer.
