Pre-test considerations
No fasting required. Collect a random stool sample in the provided clean container; do not contaminate it with urine or toilet water, and do not use stool that has touched diaper material. Keep the sample refrigerated or frozen and return it promptly, since refrigerated or thawed samples received more than three days after collection are rejected. NSAIDs can raise calprotectin by irritating the gut lining, so note recent use.

What this test is for
Fecal calprotectin is a protein released by white blood cells (neutrophils) when they migrate into the gut lining in response to inflammation. Its concentration in stool tracks the degree of intestinal inflammation, which is why it is one of the most useful non-invasive markers for inflammatory bowel disease, including Crohn's disease and ulcerative colitis. A high result points to organic inflammation in the bowel, often many times the normal level, and usually leads to endoscopy and biopsy; a low result makes IBD unlikely and supports a functional diagnosis like irritable bowel syndrome. It is most useful for distinguishing IBD from IBS in people with persistent diarrhea, abdominal pain, or change in bowel habits, since symptoms alone rarely separate the two. It is also used to monitor known IBD, where a falling level reflects mucosal healing and a rising level can signal a flare before symptoms return.
