Pre-test considerations
Results are most meaningful after three days on a consistent low-calcium, neutral-ash diet before and during collection. Where medically safe, drugs that alter mineral metabolism (antacids, phosphates, diuretics including thiazides, glucocorticoids, carbonic anhydrase inhibitors, and anticonvulsants) are often paused two to four weeks beforehand, but only under clinician guidance. Collection is a full 24 hours: discard the first morning void, then collect every drop including the same time the next morning, into a container with acid preservative. Record exact start and stop times and total volume; an incomplete collection invalidates the result.

A 24-hour urine calcium test measures the total amount of calcium your kidneys excrete in a full day, which reflects the balance of dietary intake, intestinal absorption, bone turnover, and renal handling, all governed largely by parathyroid hormone and vitamin D. High excretion shows up in primary hyperparathyroidism, sarcoidosis, vitamin D excess, prolonged immobilization, and idiopathic hypercalciuria, the leading metabolic driver of recurrent calcium kidney stones. Low excretion points toward familial hypocalciuric hypercalcemia (where this test is essential to separate it from hyperparathyroidism), thiazide use, vitamin D deficiency, or hypoparathyroidism. Because it captures a full day rather than a single moment, it smooths out the swings a spot urine or blood draw can miss, which is why it is used to evaluate stone formers, monitor bone disease, and follow people on calcium therapy for osteopenia.
