Bone-specific alkaline phosphatase (BSAP) is an enzyme released by osteoblasts, the cells that build new bone, so it serves as a specific index of bone formation. Levels rise when bone turnover is high, as in osteoporosis treatment, Paget's disease of bone, healing fractures, vitamin D deficiency, hyperparathyroidism, and certain bone metastases. BSAP is one of the most useful markers for following whether antiresorptive or bone-building therapy is changing bone activity, often shifting months before bone density scans show a measurable difference. Unlike total alkaline phosphatase, which also reflects the liver, BSAP isolates the bone fraction, which matters when liver enzymes are also elevated. It is a turnover marker, not a measure of bone density, so it is read alongside a DEXA scan rather than in place of one.

Where do I get tested?
Choose an available partner lab at checkout. If your cart contains multiple blood tests, Mito helps consolidate the eligible tests at one lab when possible. At-home kits and scans follow the collection or appointment process shown on their product page.
Is this eligible for HSA/FSA?
Yes. Eligible tests can be paid for with an HSA or FSA card at checkout.
When will I get my results?
