Pre-test considerations
No fasting is required for total bilirubin alone. The sample is best protected from light, since prolonged light exposure lowers the measured value. Heavy alcohol use, fasting, and recent strenuous exercise can transiently raise bilirubin, especially in people with Gilbert syndrome.

What this test is for
Total bilirubin measures the yellow pigment produced when old red blood cells break down and the liver clears it through bile. High levels point to liver disease (hepatitis, cirrhosis), blocked bile flow (gallstones, biliary obstruction), or accelerated red-cell breakdown (hemolysis), and visibly high levels cause jaundice and dark urine. A common benign cause of mildly elevated total bilirubin is Gilbert syndrome, an inherited variation in bilirubin processing that is harmless and needs no treatment.
This is the total measurement, the sum of conjugated (direct) and unconjugated (indirect) bilirubin. When total is high and the cause is unclear, the Fractionated test splits it into direct and indirect to separate a bile-flow or liver problem from red-cell breakdown, and the Direct test isolates the conjugated portion.
