What this test is for
Direct (conjugated) bilirubin is the form the liver has already processed and prepared for excretion into bile. When it rises, the issue is usually downstream of processing: the liver cannot excrete bilirubin normally or bile flow is obstructed, as in cholestasis, hepatitis, bile duct blockage, or hepatocellular dysfunction. It is commonly checked when jaundice, dark urine, pale stools, itching, or abnormal liver enzymes need explaining. A high direct fraction points toward liver or biliary disease, whereas elevations driven mainly by the indirect fraction point toward red blood cell breakdown or Gilbert syndrome. This is the conjugated-portion measurement specifically; total bilirubin reports the combined value, and the fractionated test reports total, direct, and indirect together when you want the full split.
