What this test is for
Beta-2-microglobulin is a small protein shed by nearly all nucleated cells, filtered by the kidneys and largely reabsorbed in the proximal tubule. Serum levels rise when glomerular filtration falls, making it a sensitive index of kidney function and tubular damage from conditions like heavy metal exposure or drug toxicity. It also climbs with high cell turnover, so it is widely used as a prognostic and staging marker in multiple myeloma, lymphoma, and other lymphoproliferative disorders, and as an early signal of kidney transplant rejection. Levels also rise in chronic inflammatory and autoimmune disease such as lupus and Sjogren's, which is why the result is read against the clinical reason for testing.
Despite the shared name, this is unrelated to the Beta-2-Glycoprotein I antibody tests (which assess clotting and antiphospholipid syndrome) or the hCG beta subunit test; those measure entirely different proteins.
