Pre-test considerations
No fasting required, and timing during the day does not matter. A recent infection can cause a transient positive, so a positive result is confirmed with a repeat test at least 12 weeks later. Note any anticoagulant therapy, which is relevant to interpreting the broader workup.

What this test is for
Beta-2-glycoprotein I is a blood protein (also called apolipoprotein H) that serves as the key target antigen in antiphospholipid antibody syndrome (APS), an autoimmune condition that promotes abnormal blood clotting. This test measures IgM antibodies against it, one of the antibodies used in the formal classification criteria for APS alongside lupus anticoagulant and cardiolipin antibodies. A positive result points toward an autoimmune cause behind venous or arterial clots, strokes at a young age, or recurrent miscarriage. Beta-2-glycoprotein I antibodies are more specific than cardiolipin antibodies, which can turn positive in syphilis and other infections.
For most APS evaluations the IgG antibody carries the strongest weight, while the separate IgA form is reserved for cases where IgG and IgM are negative but suspicion remains. APS classification requires a positive antibody confirmed on a repeat test at least 12 weeks later, since transient positives occur after infection.
