What this test is for
Beta-2-glycoprotein I (also called apolipoprotein H) is the main protein that antiphospholipid antibodies actually bind, which makes it the central antigen in antiphospholipid syndrome (APS). This test measures IgG antibodies against it, the class most strongly tied to clinical events such as venous and arterial blood clots, stroke at a young age, and recurrent miscarriage or late pregnancy loss. A positive result points toward APS and is more specific than cardiolipin antibody testing, which can also turn positive in syphilis and other infections. Diagnosis requires persistence: a positive result is confirmed by repeat testing about 12 weeks later, since transient positives occur with infection and acute illness. It is typically ordered alongside the IgM antibody and lupus anticoagulant to build the full antiphospholipid picture.
The IgG, IgM, and IgA versions of this antibody test the same antigen in different antibody classes. IgG carries the strongest association with clotting and pregnancy complications and is the usual first choice; IgM adds sensitivity, and IgA is reserved for cases with high clinical suspicion but negative IgG and IgM.
