Pre-test considerations
No fasting required. Whole blood is collected in an EDTA (lavender-top) tube; a tissue biopsy from an involved site is an alternative and often higher-yield specimen. Sampling early in active, symptomatic infection improves detection, since DNA levels can be low.

What this test is for
This test uses PCR to detect Bartonella DNA in blood or tissue and distinguishes between the two species that most commonly cause human disease: B. henselae, the cause of cat scratch disease, and B. quintana, the cause of trench fever and a driver of culture-negative endocarditis. It is a highly specific way to confirm active infection when symptoms such as prolonged fever, swollen lymph nodes, fatigue, night sweats, or unexplained endocarditis point toward Bartonella. Because the bacteria circulate at low levels, a negative result does not rule out infection: it means no Bartonella DNA was found in the sample tested, and serology or clinical correlation may still be needed. A positive result confirms the organism is present and should be interpreted alongside symptoms and other established tests. PCR is most decisive when a sample is taken from an actively involved site, such as a lymph node biopsy.
