ADMA (asymmetric dimethylarginine) and SDMA (symmetric dimethylarginine) are naturally produced molecules released when proteins break down. ADMA inhibits nitric oxide synthase, the enzyme that keeps blood vessels relaxed and healthy, so a high ADMA level reflects reduced nitric oxide availability and endothelial dysfunction, an early step in atherosclerosis and hypertension. SDMA does not block nitric oxide directly but is cleared almost entirely by the kidneys, which makes it a sensitive marker of declining kidney function and an indirect contributor to ADMA buildup. Together they are most useful in people who already carry several cardiovascular risk factors and want a view of vascular health that standard lipids and blood pressure do not capture.
