Pre-test considerations
Fasting is not strictly required for ApoB or ApoA-1, but a 9 to 12 hour fast is commonly advised for cleaner, more comparable results, especially if triglycerides are elevated. Draw at a consistent time and condition when tracking change over time. Acute illness or recent inflammation can temporarily shift the values.

What this test is for
This panel measures three things: apolipoprotein B (ApoB), which counts every atherogenic particle that drives plaque (LDL, VLDL, Lp(a)); apolipoprotein A-1 (ApoA-1), the main structural protein of protective HDL particles; and the ApoB/ApoA-1 ratio, which weighs the harmful particle burden against the protective one in a single number. People with the same LDL cholesterol can carry very different ApoB particle counts, so this panel often reveals risk that a standard lipid panel underestimates. A high ApoB or a high ratio points to elevated atherosclerotic risk, while a robust ApoA-1 reflects a stronger protective fraction. It is particularly useful when triglycerides are high or cholesterol numbers look borderline, since those are exactly the situations where particle count and cholesterol mass diverge.
If cardiovascular disease runs in your family, the ratio captures both sides of the risk balance that routine cholesterol testing reports only indirectly.

ApoB counts the actual number of atherogenic particles (LDL, VLDL, and their remnants) circulating in your blood, since each one carries exactly one ApoB molecule on its surface. It's a more accurate readout of cardiovascular risk than LDL cholesterol, because LDL-C measures the cholesterol cargo inside those particles but misses how many particles are actually out there depositing it into artery walls. Testing it matters for anyone building a baseline on heart disease risk, tracking response to diet or statin therapy, or with a family history of early cardiovascular events despite a normal-looking LDL.

Where do I get tested?
