No fasting required and no specific timing needed. This is a single blood draw. A weak or low-titer positive is common in healthy people and rises with age, so the result is interpreted alongside your symptoms; a higher titer (1:160 or above) is more likely to reflect active disease.

What this test is for
Antinuclear antibodies target the body's own cell nuclei and are the standard starting point when an autoimmune connective tissue disease is suspected. This version uses indirect immunofluorescence (IFA), the reference method, and reflexes a positive result to a titer (how strong) and a pattern (which structures the antibodies hit), both of which narrow the diagnosis. A positive result is linked to systemic lupus erythematosus, scleroderma, Sjogren's syndrome, mixed connective tissue disease, and CREST, with patterns like homogeneous, speckled, nucleolar, and centromere pointing toward specific conditions. A negative result is strong evidence against SLE, which is much of why the test is ordered. Read it against symptoms: about 5% of healthy people and up to 18% over age 60 have a low-titer ANA without disease, so a weak positive alone means little.
This IFA test reports a titer and a fluorescence pattern. The related multiplex reflex panel instead identifies specific antibodies (such as dsDNA, SS-A/Ro, SS-B/La, Smith, and RNP) when the screen is positive, and is the follow-on when you need to know exactly which antibody is present.
