What this test is for
Anti-Smith (anti-Sm) antibodies target small nuclear proteins involved in RNA processing, and they are among the most specific markers for systemic lupus erythematosus (SLE). A positive result strongly supports lupus, but anti-Sm appears in only about 20 to 30 percent of lupus patients, so a negative result does not rule it out. This panel also reports related extractable nuclear antigen antibodies (RNP, double-stranded DNA, SS-A 52, SS-A 60, and SS-B), which help characterize lupus and overlapping conditions such as mixed connective tissue disease and Sjögren's syndrome. It is typically used to investigate symptoms like persistent joint pain, fatigue, unexplained rashes, photosensitivity, and recurrent fevers.
The broad antinuclear antibody (ANA) test is the usual first screen for autoimmune disease; this panel is the more specific follow-up ordered when ANA is positive and lupus is on the table.

Where do I get tested?
