What this test is for
Androstenedione is a steroid hormone made in the adrenal glands and gonads that serves as a common precursor to testosterone and the estrogens. It has roughly a tenth of testosterone's androgenic potency, but elevated levels point to where excess androgen is coming from and are commonly checked in women with hirsutism, acne, irregular or absent periods, and other signs of hyperandrogenism, especially when polycystic ovary syndrome (PCOS) is suspected. It is also used in the diagnosis and monitoring of congenital adrenal hyperplasia (CAH), where it rises alongside 17-hydroxyprogesterone; for CAH, 17-hydroxyprogesterone is the primary marker and androstenedione adds supporting information. Because it sits upstream of both testosterone and estradiol, an elevated result is interpreted alongside those hormones, DHEA-S, and LH/FSH to localize the source. Mass spectrometry (LC/MS-MS) measurement gives the accuracy needed at the lower concentrations seen in many patients.
