Alpha-1-acid glycoprotein (also called orosomucoid) is a protein made by the liver that rises as part of the acute-phase response. Levels increase with active inflammation, infection, tissue damage, recent surgery, and some cancers, which makes it useful for tracking inflammatory activity over time. It is one of the proteins that migrates in the alpha-1 zone on serum protein electrophoresis, and it also binds and influences the availability of certain medications in the blood. It is a nonspecific marker, so a high result signals an inflammatory or stress state rather than pointing to one cause, and it is read alongside symptoms and other inflammatory markers. Despite sharing the alpha naming, it is a distinct protein from alpha-1-antitrypsin and alpha-2-macroglobulin, each of which is measured and interpreted separately for its own clinical questions.
