Pre-test considerations
Fasting is required; non-fasting samples are rejected, partly because intestinal ALP rises after a fatty meal in people with type B or O blood. This is a serum or lithium-heparin plasma draw, so EDTA, citrate, or oxalate tubes are not accepted.

What this test is for
Alkaline phosphatase (ALP) is an enzyme made in several tissues, mainly the liver, bone, and intestine, and the total ALP you see on a standard panel is a blend of all of them. This test uses electrophoresis to separate that total into its liver, bone, and intestinal fractions, which answers the question a single ALP number cannot: where the elevation is coming from. A high liver fraction points to cholestasis, biliary obstruction, hepatitis, fatty liver, or drug-induced liver injury, and often rises early before other liver tests move. A high bone fraction reflects increased osteoblast activity and is linked to Paget disease, osteomalacia, healing fractures, and bone tumors, though it is also normally higher in growing children and adults over 50.
This is the follow-up test for an unexplained high ALP, not a routine screen. If you have not yet measured total ALP, the standard Alkaline Phosphatase test or a Hepatic Function Panel is the place to start; this isoenzyme breakdown is what you order once that number comes back elevated and the source is unclear.

Where do I get tested?
Choose an available partner lab at checkout. If your cart contains multiple blood tests, Mito helps consolidate the eligible tests at one lab when possible. At-home kits and scans follow the collection or appointment process shown on their product page.
