Pre-test considerations
Fasting is not strictly required, but ALP is best drawn after an overnight fast, since intestinal ALP can rise in the hours after a fatty meal in certain blood types. ALP is also naturally higher during periods of bone growth and in pregnancy, which is normal and not a sign of disease.

What this test is for
Alkaline phosphatase (ALP) is an enzyme concentrated in the liver, bile ducts, and bone, with smaller amounts in the intestine and kidneys. High ALP most commonly points to cholestasis or bile duct obstruction (where it tends to rise more than ALT and AST) or to increased bone turnover from Paget's disease, healing fractures, vitamin D deficiency, or osteomalacia; mildly low ALP is less common and is seen with zinc deficiency, malnutrition, or hypophosphatasia. Because the same number can come from either source, an isolated elevation often raises the question of liver versus bone.

If it comes back high and the origin is unclear, the Alkaline Phosphatase Isoenzymes test separates liver from bone fractions, and the Bone-Specific Alkaline Phosphatase (BSAP) test isolates the bone signal for tracking metabolic bone disease.

Every test shows the member price next to the standard non-member price, so you can see what membership saves you. The member price is our cost, covering the lab and what it takes to run the service. It is never a profit on the test itself. Mito makes its money on the $9 membership, not on marking up your tests. Membership is $9 per month, and any applicable lab order or draw fee is itemized before you pay.
Where do I get tested?
Choose an available partner lab at checkout. If your cart contains multiple blood tests, Mito helps consolidate the eligible tests at one lab when possible. At-home kits and scans follow the collection or appointment process shown on their product page.
Is this eligible for HSA/FSA?
Yes. Eligible tests can be paid for with an HSA or FSA card at checkout.
When will I get my results?
Your results appear in your Mito dashboard after the laboratory completes processing. A clinician then reviews them before your analysis and personalized action plan are finalized. Timing varies by test, and specialty assays and at-home kits may take longer.
