Pre-test considerations
No fasting is required. A blood sample is drawn from a vein. Hemolysis falsely raises the result because red cells contain aldolase, so the lab handles the sample carefully; vigorous recent exercise or intramuscular injections can also transiently elevate muscle enzymes.

What this test is for
Aldolase is an enzyme involved in glucose metabolism, concentrated in skeletal muscle and released into the blood when muscle tissue is damaged. Elevated levels point to muscle disease: the muscular dystrophies, and the inflammatory myopathies dermatomyositis and polymyositis, where aldolase and creatine kinase together track disease activity and help guide tapering of steroid therapy. Levels can climb 10 to 15 times normal in early dystrophy when muscle mass is still intact, then fall as advanced muscle wasting sets in. The enzyme is not specific to muscle, so elevations also appear with hepatitis and other liver disease, myocardial infarction, and pancreatitis. Notably, aldolase stays normal in muscle weakness caused by nerve problems, such as multiple sclerosis or myasthenia gravis, which helps separate muscle disease from neurological causes.
