Pre-test considerations
No fasting is required for ALT alone. Strenuous exercise and some medications and supplements can transiently raise the result, so note recent heavy workouts and anything you take regularly. For tracking, keep conditions consistent between draws.

What this test is for
Alanine aminotransferase (ALT) is an enzyme concentrated in liver cells, released into the blood when those cells are injured or inflamed. It is one of the most specific blood markers for liver damage and is more sensitive to hepatocyte injury than to bile-duct obstruction. Elevated ALT shows up in viral hepatitis, fatty liver disease, alcohol-related liver injury, cirrhosis, and drug- or supplement-induced liver stress, and it is often the first lab abnormality flagged before symptoms appear. ALT is usually read alongside AST: high ALT with negative hepatitis serology can point to fatty liver, mononucleosis, or medication effects, though some people with active liver disease still have ALT in the normal range.
