What this test is for
11-Deoxycortisol, also called Compound S, is the immediate precursor that the enzyme 11-beta-hydroxylase converts into cortisol in the adrenal glands. When that enzyme is deficient, 11-deoxycortisol builds up, which is why an elevated level is the key marker for 11-beta-hydroxylase deficiency, the second most common cause of congenital adrenal hyperplasia. It also helps evaluate primary adrenal failure and secondary (pituitary ACTH) adrenal insufficiency, and it is the readout used in the metyrapone test of pituitary-adrenal reserve. Results are interpreted alongside cortisol, ACTH, 17-hydroxyprogesterone, and androgens rather than on their own.
